Liability Release / Waiver - Girls Rock
Thank you for registering! By registering, I agree that I am voluntarily participating in a Girls Rock Activity, and that I am aware of the risks associated with participating.
In consideration of Girls Rock permitting me to participate in the activity at the date and location specified on my registration (“Activity”) I agree to waive, release and forever discharge Girls Rock, its representatives, sponsors and hosts (collectively the “Releasees”) from all claims related to my participation in the Activity. I hereby agree to accept all inherent risks in this Activity, including property damage, personal injury, illness or death.
These claims include but are not limited to claims resulting from the negligence or lack of care of the Releasees and/or other participants or bystanders to the Activity, which I and/or my representatives, heirs or successors may have concerning any injury or damage I may incur relating to my participation in the Activity. I understand that these injuries or outcomes may arise from my own or other’s actions, inaction, carelessness or negligence. Nonetheless, I assume all related risks, both known and unknown to me, of my participation in the Activity.
I further agree to defend, hold harmless, and insure (indemnify) the Releasees against all claims arising out of or in any way directly or indirectly connected to my participation in the Activity. As a condition of my participation, I am aware and understand that I must have health insurance. If I need medical treatment, I agree to be financially responsible for any costs incurred as a part of such treatment.
This release and waiver shall be governed by the laws of the State of California. I understand that this waiver is intended to be as broad and inclusive as permitted by applicable laws, and agree that if any portion is held invalid, the remainder of the waiver will continue to be in full legal force and effect. I further agree that the exclusive venue for any legal proceedings related to this release and waiver shall be binding arbitration conducted in Santa Cruz County, California.
PHOTO/VIDEO: I also agree to allow Girls Rock to use any photo or video taken of me at this event for promotional purposes. I release Girls Rock from all liability resulting from using my likeness (e.g. pictures, video).
COVID & COMMUNICABLE DISEASE: I certify that I have not recently had a fever or other cold or flu-like symptoms, I have not been in contact with anyone in the last 14 days diagnosed with or who has had symptoms of COVID-19. I acknowledge that COVID-19 is a highly contagious disease that spreads from person to person and can cause death. While preventative measures can reduce disease spread, these offer no guarantee that I will avoid infection. I understand that because mountain biking may involve injury, and because close proximity may be needed to attend to injury or repair bikes, this Activity may result in an elevated risk of disease transmission, including COVID-19.
I am 18 years or older. I affirm or declare that I am freely signing this agreement. I fully understand that by signing this form, I am giving up legal rights and/or remedies that may be available to me. I understand the legal consequences of agreeing to the terms of this release of liability as a requirement of participation.

GUARDIAN CONSENT & INDEMNIFICATION AGREEMENT: I, the minor’s parent and/or legal guardian, understand the nature of the above-referenced obligations and responsibilities. On behalf of minor, I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS Girls Rock and its affiliates from all liability, claims, demands, losses, or damages on the minor’s account pursuant to this Release.
